WHAT RESULTS YOU WILL BE RESPONSIBLE FOR:

  • Overseeing the client's care communicating with the caregivers, client/client’s point of contact and health care team to problem solve, set clear expectations, and to collaboratively problem-solve issues as they arise.
  • Supervising caregivers to ensure they address the client’s companion, homemaker, and personal care needs per the established care plan.
  • Monitoring and updating non-medical changes in the client care plans, by communicating care plan changes to caregivers and their client, their client’s point of contact, and (as needed) with their healthcare team.
  • Collaborating with the Scheduler to ensure clients are matched with the right caregivers and HR for the interview process of prospective caregivers.
  • Collaborating with the COO to identify, coordinate, or provide ongoing training that increases one skill per quarter for all agency caregivers.
  • Conducting performance reviews and implements and tracks any caregiver related disciplinary actions.

THE EXPERIENCE YOU NEED:

  • Bachelors of Science in Social Working
  • 5 + Years experience working as a case manager in a Healthcare setting
  • 5+ Years’ experience working in a fast-paced office environment while maintaining a positive work culture.
