1. Name: CT Senior Resources Agency on Aging

   Purpose: Senior Resources is a private non-profit organization authorized by the Older Americans Act of 1965 and state statute and they serve the needs of older persons as a focal point and resource center.
    
2. Name: Alzheimer's Association

   Purpose: Senior Resources is a private non-profit organization authorized by the Older Americans Act of 1965 and state statute and they serve the needs of older persons as a focal point and resource center.
    
3. Name: Connecticut Coalition On Aging
   Website: http://www.coalitionagingct.org/
   Purpose: The Connecticut Coalition on Aging (CCOA) has, for more than 40 years, been a high profile, statewide advocacy and education resource representing the interests and concerns of CT’s aging residents, particularly the frail elderly. 
    
4. Name: Connecticut Center For Patient Safety
   Website: http://www.ctcps.org/
   Purpose: The Connecticut Center for Patient Safety works in our communities, within our healthcare systems, and with elected officials to improve the quality of healthcare and to protect the rights of injured patients through education, accountability, and advocacy.
